The 2009 Open Enrollment packets were recently sent to all participants. To view a copy of the 2009 Open Enrollment Packet click here.
All changes made during the Open Enrollment period will be effective January 1, 2009.
Participants who are currently enrolled in the Diamond, Platinum, or Hospital Supplement plan
If you wish to keep your current coverage for the 2009 calendar year, you do not need to return anything to the Health Trust.
If you wish to:
- Change plans
- Add/Delete coverage for your dependent(s)
- Change your primary or contingent beneficiary
- Enroll in the Section 125 Premium–Only–Plan (only if you are not currently enrolled)
You MUST complete a change form, submit the appropriate eligibility documents, and return it to the Health Trust by 5:45 p.m. on November 25, 2008.
If you are adding a domestic partner to your plan
If you are adding a domestic partner, you will complete a Change Form and a Declaration of Domestic Partnership form. To download a copy of the Declaration of Domestic Partnership form, visit the Trust Forms page by clicking here.
