To obtain your FREE glucose monitor, you must be diagnosed with diabetes and be covered by one of the Teachers Health Trust plans. Click here to e–mail the Wellness Division, and a representative will contact you to schedule an appointment for in service on the usage of your free glucose monitor.

MAKE SURE YOU TAKE ADVANTAGE OF THE FOLLOWING PROGRAMS!

Participants on either the DIAMOND or PLATINUM plans are entitled to six (6) outpatient visits per Calendar Year for Nutritional Consultations with an In–Network Registered Dietician. This benefit is not limited by diagnosis or Routine Preventative Care Maximums. No prior authorization is required.

Nutritional Consult $20 copayment for Diamond and $30 copayment for Platinum.

Diabetic Education $0 copayment for either plan.

Utilizing Medco Health mail order pharmacy can save you time and money

  • Ask your doctor to write two (2) separate prescriptions: the first for up to a 30–day supply; the second for a 90–day supply with up to a year supply of refills.
  • Fill the 30–day prescription at your retail pharmacy so that you have at least a 30–day supply of your medication on hand.
  • Submit your 90–day prescription to the mail order program by one on the following methods:
    • Doctor’s fax: Give your ID number to your doctor and ask him or her to call 1–888–327–9791 to obtain fax instructions; or
    • Mail: Mail your prescription, the required copayment and order form to Medco. Mail order forms are available online at www.medcohealth.com or by calling Medco at 1–800–818–2364.
  • Allow up to 14 calendar days for your medication to arrive in the mail.
Important Note
Do not send a prescription for a 30–day supply with refills to the mail order program. Medco can only fill the prescription for the quantity limit written on the prescription; therefore, you will be charged the 90–day copayment for the 30–day supply of medication. Your prescription must be written for a 90–day supply in order for the mail order program to save you money.
