Medical and Dental Providers
The search function on the right side of the screen you will find in-network providers for the Performance Plus plan. You've already pre-selected your Patient Centered Medical Home (PCMH) Primary Care, Pediatrician, and OB-Gyn. Your selected PCMH doctor is your personal provider to actively work together with you to manage your healthcare needs. We understand that on occasion, some patient-doctor relationships are not an exact match. You are allowed to change your PCMH doctor(s). It is important to note that the systems and processes that make the PCMH model possible are complex; changing a provider is not as simple as clicking a button - it requires the linking of multiple systems, platforms, and communication systems.
If you need to change your PCMH doctor, you may do so once each quarter with the change being effective the following quarter. Simply call a WellHealth Healthcare Advocate at 855-404-9355, answer a few questions, and the change will be made effective the following quarter. A completed Provider Change Request Form is also required via email in order to expedite the process. Simply fill out this form and email to advocates@wellhealthqc.com. You do not need to call and email. Both options are available for your convenience.

What is PCMH?
The medical home model holds promise as a way to improve health care in America by transforming how primary care is organized and delivered.
The medical home model encompasses four key functions and attributes:

1. Comprehensive Care
The Patient Centered Medical Home model is designed with the patient's complete care in mind and takes an approach that looks at individuals holistically. In other words, patient care is directed at wellness and prevention, and not just when acute or emergent care is needed. Providing comprehensive care means looking at the whole person as opposed to and single part and enabling providers to partner together in a patient's total care.

2. Patient-Centered
Patient-centered care is care that is driven by the patient's values and needs. The PCMH model seeks to strengthen the patient's control of their healthcare though encouraging providers to listen to and develop relationships with their patients so that decisions are made with the patient's values at heart. Patients are therefore better able to play an active role in their wellbeing through managing and organizing their care hand-in- hand with their providers.

3. Coordinated and Accessible Care
With a Patient Centered Medical Home model in place, primary care providers now have the ability to coordinate between all elements of the healthcare system, including specialty care, hospitals, home health, behavioral health, and community services. This coordination is made possible by improving accessibility and communication between these entities. In addition, the PCMH model requires participating providers to provide better access to care, meaning scheduling appointments sooner with shorter wait times, 24/7 access to virtual care, and enhanced in-person visits.

4. Safety and Quality
The PCMH model demonstrates a commitment to quality through ongoing engagement in evidence-based medicine, clinical support tools, and shared decision making with patients. Providers are given performance measures and quality standards and held accountable for their fulfillment or disregard for those goals. This data then acts as motivation to improve their overall performance, ultimately improving the level of quality and safety for patients.

Thank you for the patience you have shown throughout these past few months during the transition to your Teachers Health Trust Performance Plus Health Plan. A number of changes have been made in a very short time, and your cooperation is greatly appreciated. We all have the same goal to provide the best and most efficient medical care to you and your families.
One of the exciting changes to the health plan is the addition of the Patient Centered Medical Home (PCMH), which places your total medical care into the hands of your caregiver, as well as yourself, and results in a healthy and active relationship with your PCMH provider. When you select your Patient Centered Medical Home doctor(s), you are choosing a personal partner to manage your healthcare needs. However, we understand that on occasion some patient-doctor relationships are not an exact match. The program allows you to change any of your Patient Centered Medical Home Doctors. However, the systems and processes that are in place, which make the PCMH model possible, are complex; changing a provider is not as simple as clicking a button - it requires the linking of multiple systems, platforms, and communications systems.
Therefore, the option of changing your PCMH provider at will is not possible. If you need to change your PCMH doctor, you may do so once each quarter, with the change being effective the following quarter. Simply call a WellHealth Healthcare Advocate at 855-404-9355, answer a few simple questions, and the change will be made effective the following quarter. A Provider Change Request Form is also attached for you to complete and email in order to expedite this process. Simply fill out this form and email to advocates@wellhealthqc.com. You will not need to both call and email; you can choose whichever option is more convenient for you.
A number of you were unable to select a PCMH provider of choice during the open enrollment period. In some cases, your primary care provider was "pending" or not yet on the network at all. If that is what has occurred in your case, you have been assigned a PCMH provider. If this applies to your situation, you may call or email a Healthcare Advocate to obtain the name of your assigned provider and/or change your PCMH provider.

Teachers Health Trust Performance Health Plan members now have the ability to access medical and behavioral health providers 24 hours a day, 7 days a week through a Telemedicine platform powered by MDLIVE.
Providers are able to diagnose and treat conditions such as the cold and flu, fevers, strep throat, conjunctivitis (pink-eye), sinus infections, rashes and more!
In addition, members have access to behavioral health professionals to help them with their immediate health needs.

Yes, each dependent needs to have a unique username for billing purposes. However, they can be registered under the same email address. Once you have registered as the primary account user, you can click on the "My Account" tab and register any dependents by verifying their information and creating their username.
The goal of telemedicine is to give you the ability to see a doctor within minutes. However, there could be a delay based on the amount of members trying to access care at the same time. You do have the ability to schedule an appointment with any of the doctors available and see them when it is convenient for you.
Yes. You can download the MDLIVE app, login, and start to access care. You can download through the App Store or Google Play.
Yes. Prior to your visit, you will select your pharmacy (verify that you are selecting an in-network pharmacy). If a medication is prescribed by your doctor, you will be able to pick up the medication the same day in most cases.
