Welcome to Open Enrollment 2019


You will have until November 9, 2018, to log into the online enrollment site, review your plan and make any changes needed. All changes will become effective January 1, 2019. All required supporting documents (marriage certificates, birth certificates, etc.) must be received by the Trust no later than December 15, 2018. Any documents received after this date are subject to a $100 administration fee. Please note that no supporting documentation for open enrollment will be accepted after December 31, 2018.

During open enrollment, you may supplement your health plan with additional benefits through American Fidelity such as:

  • Accident Only Insurance
  • Cancer Insurance
  • Long-Term Disability Income Insurance
  • Short-Term Disability Income Insurance
  • Flexible Spending Account
  • Life Insurance

Plan Selection

For more information regarding the plan benefits, we encourage you to review the following documents:

  • Medical Benefits - Copayments at a Glance: This medical benefit summary provides important copayment and coinsurance information for the most commonly utilized medical benefits.
  • Prescription Drug Benefit: Information regarding in-network, out-of-network and mail order pharmacies.
  • Dental Benefit: Learn more about your benefits that cover preventive, basic and major dental services.
  • Vision Benefit: Look here for copay information regarding exams, lenses and frames.
  • Premium Rate Sheet: Information regarding premium amounts for both active and retired CCSD employees.
    • If you are a retiree, shared contract, or self-pay participant in the plan, the premiums in the enrollment application may not be the actual premium you will be charged for the new plan year. Contact member services at 702-794-0272 for premium information.

 

Section 125: Pre–Tax Paycheck Deduction

You may make the election to have health insurance premiums deducted as either pre-tax or post-tax under the Section 125 Premium Only Plan (POP). Note: If you have a domestic partner enrolled on your plan, you will be ineligible to enroll in Section 125 POP.
